How do I transfer interest due to the death of an owner with a will?
If the owner had a will and it was or will be probated in the state of mineral interests, please provide a copy of the following:

  • Death Certificate
  • Letters Testamentary or Letters of Administration (not given in Muniment of Title)
  • Order Admitting Will to Probate
  • Final Decree (for Oklahoma probate)
  • Will

How do I transfer interest due to the death of an owner without a will?
If the owner did not have a will or will not be probating a will please provide a copy of the following:

  • Death Certificate
  • A completed Affidavit of Heirship (PDF) (It is strongly recommended to file in the county of mineral interests) 
  • The Will

How do I change my address?

Complete and return the Change of Address form (PDF) to the address, email or fax provided on the form. Please allow at least four weeks for the update to be processed

What is needed when there has been a purchase or sale of interest?

Provide a copy of the conveyance (such as a deed or assignment) filed of record in the county or parish where the property is located

What is needed for personal name changes?

Provide a copy of the court order supporting the requested name change (such as a divorce decree which reinstated the owner’s maiden name) or a copy of a marriage certificate

What is needed for corporate name changes?

  • Provide a copy of the Certificate of Name Change and submit a new W-9 form if a company or corporation has changed its name
  • Provide a copy of the Certificate of Merger and submit a new W-9 form if a company or corporation has changed its name due to a merger

What is needed for Powers of Attorney, Agents and Legal Guardianships?

  • Provide a copy of the Power of Attorney if an owner has appointed an attorney-in-fact
  • Provide a copy of the Agency Agreement if an owner has appointed an agent
  • Provide a copy of the Letters of Guardianship if an owner has been declared by a court to be incompetent or incapacitated or is otherwise deemed by a court to require a court-supervised legal guardian

What is needed for Trusts?

  • Upon the creation of a trust, provide a copy of the trust agreement (or the Memorandum of Trust) and the conveyance which transferred the interest to the trust and which has been filed of record in the county or parish where the property is located
  • Upon the termination of a trust, provide a copy of the Dissolution of Trust and the recorded conveyance which transferred the trust’s interest to the beneficiaries

Payments

When will I receive my payment?

  • Checks are mailed on the 18th of the month.
  • If you are signed up for Direct Deposit, the funds will be deposited on the 18th of the month.
  • If the 18th falls on a holiday or weekend, the payment will be moved forward or backward to the closest work day.
     

How do I sign up for Direct Deposit?
Complete and return the Electronic Funds Transfer Authorization Agreement (PDF). Follow the instructions and return to the address provided on the form.
 

I haven’t received my check yet. How do I get a replacement?
Please allow 10 business days before requesting a replacement. If you still have not received your check please call or email the Owner Relations Unit. Additionally, if you’ve moved recently please complete and return a Change of Address form (PDF).
 

My check has been lost or stolen. How can I stop it from being cashed and get a replacement?
Please call or email the Owner Relations Unit.
 

What is the value of my mineral interests?
Phillips 66 does not provide appraisals for leases. However you may contact a royalty broker or local bank in the county where the leases are located.

Account Information

Where do I find my owner number?
Your owner number can be located on your division order, check detail and in the Account Number box on your 1099Misc form. Most forms can be submitted without the owner number as long as the full Social Security Number or Tax ID Number is provided.
 

I received an Unclaimed Property letter. What do I need do?
Please complete the claim form at the bottom of the letter to the best of your ability. If you do not know the owner number, you can leave that space blank. This is time sensitive, so please return the claim form to us as soon as possible.
 

When will I receive my 1099Misc form?
1099Misc forms will be mailed at the end of January to the current address on file. If you have a new address, please complete and return a Change of Address form (PDF) before year end to ensure your address is updated in time.
